Utah’s premier New Choices Waiver case management company dedicated to helping you or your loved one experience the joy of living independently.

The New Choices Waiver serves individuals who are currently residing long term in skilled nursing or similar facilities and are receiving, or are eligible to receive, Medicaid benefits. The program is designed to empower those on the waiver to be an active participant in their Person Centered Care Plan.

It offers an option for these individuals to live independently in the community of their choice, with supportive services in place to meet their health and safety in the setting that they have chosen.

The goal is to help you find the living situation you want, ensuring that you live in a safe and supported environment that meets your health and safety needs.

That’s why Advocates for Independence assists our clients in finding the best location for their health and safety with the least restrictions, creating an environment which offers each individual independent living and joy.

Along with helping clients find housing within the community of their choice, we help our clients navigate the sometimes complex social services network, including filling out forms, applying for Medicaid services such as Food Stamps, coordinating medical care and arranging transportation to medical appointments, shopping and social activities. In other words, we help our clients live as independently as they can, while ensuring that they live in a safe, clean and fully supported environment.
