Advocates for Independence is Utah’s premier New Choices Waiver case management company dedicated to helping you or your loved one experience the joy of living independently.

Designed to serve people who are living in long term skilled nursing or similar facilities (except for institutions for mental disease) and are receiving or are eligible to receive Medicaid benefits.

Must be an active resident of the facility. If a resident discharges from the facility before being accepted on the New Choices Waiver, the application will be rejected and the person must be readmitted into the facility to restart the waiting period and application process.

Designed for those with a primary physical diagnosis. Those with a primary mental diagnosis will not qualify.

The New Choices Waiver is PRIMARILY about moving people from skilled nursing to the living situation of their choice with the services of their choice that meet their needs.

It was designed to empower those on the waiver to be an active participant in their Person Centered Care Plan.

How to Qualify:

Skilled Nursing Route

Applications are accepted year round on a first come, first served basis.

You must stay 90 days in a skilled nursing facility and be medically eligible for long term Medicaid care. The application can be started after 60 days.

OR

Stay 30 days on a Medicare stay in a hospital or skilled nursing facility and be medically eligible for long term care under Medicaid at the end of the Medicare stay. The application can be started after 15 days.

How to Qualify:

Assisted Living Route

Applications are accepted only during the open application periods:

  • March 1 – March 14
  • July 1 – July 14
  • November 1 – November 14

Minimum stay of 365 days in assisted living facility. However, because generally more applications are received than there are slots, applications are ranked according to length of stay. Historically, the cutoff point has been much longer than 365 days.
